TEAM/VOLUNTEER REGISTRATION INSTRUCTIONS

The OLQP Universal Sports Registration Form can be filled out on-line and printed, or you may print a blank copy and fill it out in ink.  The registration form must be signed and returned to the OLQP Sports Committee accompanied with the appropriate registration fee.  THIS FORM MAY NOT BE E-MAILED, as it MUST be signed and submitted with a check, and must therefore be returned in person.

If you are interested in Volunteering as a Coach, Assistant Coach, Scorer, Clock Keeper, Referee or Gym Assistant, please click here for the Volunteer Registration Form.  This form may be e-mailed by clicking "Submit" after it has been filled out.
